If you get an interview, you're moving on to the second stage of the screening process.
If you don't get an interview, you've been rejected from the program.

The application is the initial screening process to prove that you have the credentials needed to be in the program.

Hi there. I have a few questions about the application that I was hoping someone here could clarify.

1. The application states to put N/A for questions that don't apply to us, and not to leave any questions blank. Does this mean that every field must have an answer or an "N/A"? For example, the Prior experience field lists 5 spots for prior teaching experience. If someone only had one, would they fill out N/A for every field in the other 5 spots? Or for Higher Education, it gives us 5 spots to enter school information. If we've only attend one university, do we fill out N/A for all the other spots? (Sorry if this is a silly question.. I'm just a little confused about how frequently to us it.)

2. For the section on Japanese related studies and foreign language skills, it states
"
If you have additional experience you would like to include, you may add attachments, provided they are in the same format as the chart in the print out version of this application and do not exceed one page (once you create the PDF of your application, you can view the chart). Please fill in the appropriate spaces on the application form first before adding any attachments."

I was wondering if someone could clarify this a little bit better? I've taken several more Japanese related courses then there are spots for, but I wasn't sure exactly how it wants us to indicate those.

I am applying from the United States.

1. Will there be a problem because my application has my new married last name, and my birth certificate has my old last name?
2. Is it okay to guess on the months I volunteered and was awarded awards. I can't exactly remember when anymore, and I have no way to look them up.
3. I want to put my spouse as my primary emergency contact, but he is applying with me, and if he gets accepted his information will change. Should I just put my parents instead?

1. You will need to provide photocopies of the name change documentation (include with all 3 application sets).
2. Yes, you can estimate that sort of date. It does not have to be exact (although neither should it be very far off).
3. For now, it's all right to put him down as an emergency contact (unless he would be accompanying you as a dependent if he is not accepted; not all married JETs do this). If that is the case, it would be best to put down a contact who will definitely be here in the U.S. If you are accepted and need to change this information at any time after submitted the application but before departure, you should contact the coordinator at the consulate you interviewed through and submit an "Alterations to Reply Form".

Self-Assessment Med form and other questions!

Hi everyone!

I'm in the process of working on my application, and I have a few scattered questions...

1) For the higher education section... I have essentially attended one university only, but I *do have one course that is a transfer credit from a university I attended over the summer as part of a research program. This course shows up on my official transcript, but it doesn't say when I took the course nor does it show the grade I got (I actually don't think it counted as a graded course). So! Do I need to include a transcript from the second school? I've ordered one anyways, so maybe I will include it just to be safe.

2) In the studies section, I assume it's asking if you've taken any history/culture courses specific to Japanese culture...? I've taken cultural anthropology, but I don't think that counts.

3) Where can we mention that we have studied Japanese on our own? There's the informal studies section, but it asks for institutions and courses, which are not applicable.

4) On the medical form... I haven't had any serious ailments other than typical sicknesses (flu/cold) and some minor injuries (neck problem/strain, basically). Should I include these? (I'm specifically talking about 2a). It also asks if you've received any prescriptions over the last 5 years. I can say I have, but they've all been for minor things like temporary infections, muscle pain... and they've been temporary. Include these too?

5) I also have Raynaud's syndrome, which basically means my fingers go numb really easily in cold weather. There's no treatment for this other than, say, dressing more warmly... I assume I should mention this? I don't know if it's even really in my medical records as it's not very serious or life-altering.

I'm sure that this has probably been answered somewhere on here but I just spend half an hour looking for it and cannot find an answer. So sorry if it's a repeated question!
I saved my PDF application and plan to print it out and send it by tomorrow or Saturday. However, the PDF cuts off a couple of my longer answers,even though they were within the limit. I really don't want to shorten the answers, everything I wrote I feel is important to answering the question however will it be acceptable for it to be cut off? Should I include a "post it" type note writing out the rest of the answers? It only did that for 5 or 6 answers.

***Edit: I think I found a similar question where you had said that some may be cut off and to send it the way it is. So then my revised question is: would it be preferable to just send the application cut off the way it is, to or attach a little post it with the answers written out?

1. Yes, I would advocate submitting the separate transcript.

2. Yes, Japan-related culture and history.

3. Under informal studies, list something like "Self-Study Japanese" under "Institution." If you can, specify your general level (i.e. introductory, intermediate, advanced). You can leave the grades section blank.

4. It sounds sort of borderline for needing to provide a Physician's Form. I would say to declare them and get the Physician's Form just in case, but it's basically up to you.

5. Yes, you should mention it and include a doctor's note. For one thing, if you are requesting a warmer climate, it provides a bit more weight to your placement request.

This is a problem with the online application. We'll be using the electronic version as well as the print-out when reviewing applications, so it's all right to send it in as is without rewriting your answers.

Just out of curiosity, and to clarify a bit, would someone automatically get disqualified for not including that aforementioned temporary infection, muscle pain, or other minor irritations of the same level? Or are these things taken on a case-by-case basis?

...if it helps, I went to my most recent physician and requested a copy of my medical records, as I didn't have them on hand. I think I'm going to say that if something is listed on my official medical history, I will mention it. I don't think that getting a cold, muscle spasm, or the like that you are no longer experiencing or that is in no way serious, should really matter in terms of whether you're eligible. If anything it will result in loads of people getting the physician's form filled out when it's not really necessary (in my opinion). Most of those things don't register in your medical records anyhow...so I guess what I'm saying is if it was important enough to be mentioned in your records, maybe that's a reason to mention it (and alas, go find a physician...not easy for recent graduates who may have lost health coverage/services upon graduation :| ) I think it would be shortsighted to disqualify people because they failed to mention a sprained finger they got 3 years ago or something, personally. BUT, I am not a coordinator, so take what I say with a grain of salt.

I have a international experience that is basically also work experience which would make it fit in both sections. Should I just put it in both sections, or should i just choose one section to include it.

If we find out that something that should have been included on the Self-Assessment Medical Form was not, that would be grounds for disqualification. Whether or not it would actually happen depends upon the situation and what the ultimate decisions from those involved would be. Something truly minor probably would not be a problem (assuming it was so minor that it's not something that would need to be included on the Self-Assessment Form), but given that people's interpretations of "minor" can vary, I am hesitant to say anything very firm about that.

Yes, you can put it in both sections. Whether or not a reviewer will count it twice is another matter, but you can list the same thing multiple times.

I have a question concerning transcripts and studying abroad. The courses that I took abroad are listed on my transcript as education abroad courses and the grade that I received is also listed. However, the name of the course is not yet available on my transcript. Would this invalidate my transcript?
