Letters of Reference guidelines

I never finished my application for the JET program last year. I'm definitely going to do it this year. If I remember correctly, there was actually a form that was to be used for the letters of reference. I think it had a place for the writer of the letter of reference to fill out their personal info and then specific questions about the applicant that they were supposed to respond to. I will be contacting some professors from my university to get them to write my letters of reference. I need to give them enough time, preferably before the semester starts in September. They get quite busy after school starts. The JET program for US citizens has not posted application info yet so I can't get this form. Does anybody have an old one that I could look at? I want to give the info to my professors as soon as possible so they can start writing the letters.

Nobody??? I know someone out there still has their application materials from last year. I'd really appreciate if someone could upload the blank letter of reference form or at least tell me what my professors are supposed to write in the letter of reference. Can someone please fill me in?

Here it's all on the Irish website. they keep it fairly up to date :)
